• Special event permit request
• Travel permit requests

Please Note: Permits are required for all formally organized team events. No permit is required for an outdoor ice practice, however the following is mandatory:

1. The practice is to be held at a community league rink and the coach/team management has made an inspection of the facility to insure the safety of the team.
2. Only on-ice assistants who are registered on the team sheet are covered by CHA insurance.
3. Players are in full equipment.
